Initial I-20/DS-2019 - Complete this request for Wake Forest University to issue you an I-20 or DS-2019. 

Transfer In I-20 - Students on the F-1/J-1 visa will need to make arrangements to have their current SEVIS record transferred to WFU. Complete this request for Wake Forest University to issue you a Transfer In I-20.
